Care Coordination Caseload Holder Lead Nurse
We are looking for a motivated, dynamic and positive band 6 nurse with excellent communication and record keeping skills to work within the care coordination team in all Claire House settings; Wirral Hospice, Liverpool Hospice and the community setting. The individual will be caring, professional and capable of working across teams and services internally and externally.
We are looking for extensive experience and knowledge within nursing care, to include effective leadership skills and with a knowledge of safeguarding procedures. The role involves supporting families holistically within their journey at Claire House, participating and chairing meetings and is the main point of contact for a large caseload of families and professionals. To build positive relationships, gain updates, work collaboratively and provide appropriate support to families in times of need.
This role is full-time and can be worked flexibly across 4 days, minimum of 30 hours is required, predominantly office hours, with an expectation to work flexibility over a 7-day period if needed, with an option to work at home at times; dependent on service need.
Key skills for this job include: Experience working with children/young people and families, excellent communication skills, excellent organisational skills, ability to lead a small team and to work autonomously. To be able to prioritise, manage time and workload, problem solve and meet deadlines. Good IT skills and a car driver is essential.
